6/30/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ben-Nita to conduct an individual counseling session. Ben-Nita appeared to be energetic and jovial. QMHP-C informed Ben-Nita that the focus of the session is learning effective coping and anger management skills to reduce stress when upset or angry. 2. Behavior: Ben-Nita presented cooperative behaviors throughout session as she was able to discuss challenges with her mother, but she knows how her mother is so she really don’t be listening to her or be worried about her. 3. Behavior: Ben-Nita actively participated in the session and needed some redirection at times as she was hostile with her mother by some of the things that she was discussing as it relates to her behaviors being presented. 1.Intervention: QMHP-C began the session by inquiring about Ben-Nita’s current thoughts, feelings and emotions. QMHP-C processed with Ben-Nita as to how she was feeling today. QMHP-C probed Ben-Nita to talk about the various emotions that she experienced since the last session was conducted with QMHP-C. QMHP-C assisted Ben-Nita with understanding the cause and effect of emotions. QMHP-C provided Ben-Nita with role playing activities relevant to various emotions. QMHP-C probed Ben-Nita to talk about ways that she can utilize her coping skills to assist her. QMHP-C assisted Ben-Nita with this task by providing alternatives in various settings inclusive of home, school and community. Ben-Nita and QMHP-C completed a worksheet to begin a series of self-actualization lessons. 2. Intervention: QMHP-C assisted Ben-Nita with interpreting her answers as well as to the triggers that cause her to loose control of her emotions. QMHP-C concluded with a review of the concepts that were learned while providing Ben-Nita with support through active listening, role playing, and summarizing what was discussed with her during today’s session. 3. Intervention: QMHP-C observed Ben-Nita and Ms. Shabazz interacting in an appropriate fashion free from any sarcastic remarks or rude behaviors. QMHP-C allowed Ben-Nita to openly discuss with QMHP-C and her mother her feelings as QMHP-C processed thoughts and feelings expressed. QMHP-C engaged Ben-Nita in an activity about her family and relationships with family members in the home to include which members she has the most conflict with, who she respects the most and closest to among others. 1. Response: Ben-Nita appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ben-Nita was receptive to QMHP-Cs’ intervention. She indicated during the session that she was still a little tired but she had energy. Ben-Nita was cooperative during the role playing activities by providing QMHP-C with feedback to the information that was being presented to her by QMHP-C during today’s session. Ben-Nita stated during the session as QMHP-C was reviewing with her different techniques on how to control her angry that she tries to use self-discipline, but sometimes she cannot control her emotions especially when she is already mad. 2. Response: She indicated that when she is really mad she will begin to cry and then she doesn’t want to hear anything that anyone has to says (indicating that she is really angry and she may slap someone). 3. Response: Ben-Nita engaged in activity noting the different relationships she has with family members such as admiring her mother the most out of her family members because of the way she takes care of her needs and wants.
